How Does ChoiceConnect Work?

Our office is launching ChoiceConnect to help you stay healthy from the comfort of your home.

  • bulletAll you need is a phone to participate.
  • bulletYou will receive weekly messages or phone calls asking about your condition specific symptoms.
  • bulletThe text messages or phone calls will arrive at a time that you choose. It only takes a few minutes each week to respond to messages.

You can get immediate help from a ChoiceConnect care manager if you respond with worsening symptoms or health concerns. ChoiceConnect will then work with you and our office to help you resolve them.
